RESPONSIBILITIES
CLINICAL CHAPLAINCY (70%)
• Provides spiritual care services to patients and their families through video technology and telephone calls.
• Assesses and responds to the psychosocial, spiritual, religious, and grief-related concerns of our patients, their families and support systems.
• Assists patients and families in accessing and using community resources.
• Mediates conflicts within families at the discretion of the IDT.
• Assists with funeral arrangements in collaboration with the IDT.
• Assesses for social work and medical needs and communicates these promptly to the appropriate IDT.
• Establishes and maintains relationships with community faith groups.
• Identifies spiritual distress, quality of life goals, spiritual interventions, and frequency of Chaplain visits to complete the spiritual support Plan of Care.
• Consults and collaborates with the IDT and others involved in the patient's care on an ongoing basis, updating the care plan as appropriate.
• Ensures that there are appropriate sacred resources to address diverse spiritual, religious and cultural needs of patients.
• Completes documentation within 24 hours of care coordination or visit.
• Educates IDT on spiritual distress screening and appropriate religious or cultural support for individual patients
• Assumes other duties in support of RC's needs as directed by the or Medical Director.
• Assists with community outreach and education regarding Resolution Care services when requested by clinical management.
• Assists with research projects as requested.
INTERNAL STAFF ACTIVITIES (20% ADMINISTRATIVE)
• Facilitates weekly staff Honor Gatherings and other staff meetings as requested.
• Provide emotional and spiritual support to employees during personal and professional life stressors
• Connect employees to additional mental health resources for ongoing, in-depth support as appropriate
• Promote connections between patient care and administrative employees to maintain continuity of mission and unity of purpose
• Supports other directors/team leaders to create and sustain healthy team cultures
• Foster an atmosphere of open dialogue between departments and to the larger organization
• Organize responses to crisis events, external or internal, which impact the organization
• Demonstrate personal and professional identity and integrity within one's scope of practice to facilitate relationships of trust
OTHER DUTIES AS ASSIGNED (10% ADMINISTRATIVE) Requirements:
QUALIFICATIONS AND EXPERIENCE
Minimum
• Masters of Divinity degree or equivalent experience
• Minimum 2 years health care chaplaincy
• Four units of Clinical Pastoral Education or equivalent
• Excellent oral and written communication skills required
• Positive interpersonal skills required
• Must have general computer skills and a working knowledge of word processing programs and the internet
Preferred
• Experience in hospice, palliative care, oncology or home health
• Board Certification as a Chaplain from BCCI, APSC, CPSP, SCA, NAJC, NACC, APC, etc.
• Palliative care certification
Position Location and Reporting
• This position is remote
• Reports to the Chief People Officer for administrative management and dotted line to the Director of Social Support Services for clinical responsibilities oversight.
