You're the person who connects patients to care - and the person who proves it's working. Encounter Telehealth, the leading rural behavioral health provider, is hiring a Care Navigation & Reporting Coordinator to anchor our RHTP 4.4A grant program, serving roughly 250 rural Nebraskans across 8 partner facilities with behavioral health and chronic disease care. This is a dual-function role. Half your time is hands-on patient navigation - coordinating appointments, closing the loop on referrals, supporting medication adherence, and helping patients work through insurance, transportation, and other barriers that get in the way of care. The other half is the data and reporting work that demonstrates the program is making a difference: quarterly DHHS submissions, community needs assessment refreshes, and outcomes tracking against grant performance measures. It's not a data entry job. It's not a pure care coordinator job. It's a clinically-grounded role for someone who understands what they're seeing in the field and can translate it into accurate, meaningful reporting. What you'll be doing On the navigation side: managing care coordination across 8 rural skilled nursing facilities and critical access hospitals, implementing closed-loop referral tracking from screening through ongoing therapy, supporting PHQ-9 and GAD-7 screening protocols alongside our clinical team, and serving as a trusted liaison for facility administrators, discharge planners, and nursing staff. On the reporting side: compiling and submitting quarterly and final reports to Nebraska DHHS, maintaining the program's community needs assessment, monitoring enrollment and screening completion against grant targets, validating clinical data from partner facilities, and working within the data infrastructure built by our CTO team. What we're looking for We're strongly preferring an RN or LPN, but we're also open to LSW/LCSW, CMA, respiratory therapists, physical therapists, or other licensed healthcare providers with the right experience. At minimum, a bachelor's in healthcare administration, social work, health informatics, or a related field with equivalent clinical experience. You should bring 3+ years in care coordination, case management, patient navigation, or a clinical role that included real documentation and reporting work. Experience with chronic disease populations (diabetes, CVD, COPD, CKD) or behavioral health is strongly preferred. Federal or state grant reporting experience is a significant plus. Tebra experience is a bonus.
