Travel Registered Nurse – RN - Hospice - PA West Territory - FT

About the position This role is for a Hospice RN – Regional Support. In this dynamic role, you’ll provide direct care to patients and support to families, while also serving as a floating clinician—temporarily assigned to different hospice branches based on census needs or staffing gaps. This role requires regional travel and may include overnight stays based on assignment needs. A company fleet car is provided for eligible employees. The role is typically Monday–Friday with occasional on-call responsibilities. Responsibilities • Deliver high-quality, compassionate nursing care for hospice patients • Manage and implement individualized plans of care • Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.) • Assess and address patients’ physical, emotional, spiritual, and environmental needs • Accurately document all care provided and ensure compliance with hospice protocols • Educate families and caregivers on care instructions, medications, and safety • Participate in the on-call rotation and provide after-hours care as needed • Collaborate with physicians, referral sources, and the care team • Assist with referrals, regulatory documentation, and payer compliance • Travel to various Gentiva hospice locations as assigned to fill urgent needs Requirements • Current, unencumbered RN license in the state of employment • Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting) • Current CPR certification • Valid driver’s license, auto insurance, and reliable transportation Nice-to-haves • Hospice and end-of-life nursing experience • Skilled in clinical assessments and compassionate communication • Strong critical thinking, flexibility, and problem-solving skills • Comfortable working in varied environments with shifting needs • Proficient in documentation and electronic health records • Calm and supportive in crisis situations; skilled at family engagement Benefits • Competitive Pay • 401(k) with Company Match • Career Advancement Opportunities • National & Local Recognition Programs • Teammate Assistance Fund • Medical, Dental, Vision Insurance • Mileage Reimbursement or Fleet Vehicle Program • Generous Paid Time Off + 7 Paid Holidays • Wellness Programs (Telemedicine, Diabetes Management, Joint & Spine Concierge Care) • Education Support & Tuition Assistance • Free Continuing Education Units (CEUs) • Company-paid Life & Long-Term Disability Insurance • Voluntary Benefits (Pet, Critical Illness, Accident, LTC)
