Job Overview
We are seeking a dedicated and energetic Physician to join our team specializing in Remote Patient Monitoring (RPM) and Chronic Care Management (CCM). Duties
• Conduct thorough patient assessments via telehealth platforms, utilizing EMR (Electronic Medical Records) systems to document findings accurately.
• Develop and implement individualized care plans focusing on chronic disease management, preventive care, and health coaching strategies.
• Monitor patient progress remotely, adjusting treatment plans as needed to optimize health outcomes and adherence.
• Collaborate with multidisciplinary teams including nurses, specialists in psychiatry, endocrinology, cardiology, and other fields to coordinate comprehensive care.
• Provide expert guidance on medication management, including IV infusion protocols, wound care, and post-operative or post-acute care planning.
• Ensure compliance with HIPAA regulations and maintain patient confidentiality at all times during telehealth interactions.
• Stay current with clinical research developments and incorporate evidence-based practices into patient care strategies.
Qualifications
• Medical degree (MD or DO) from an accredited institution with valid licensure to practice medicine.
• Proven experience in outpatient or inpatient settings with a strong background in medical office procedures and medical terminology.
• Familiarity with EHR/EMR systems such as Epic or similar platforms; experience with telehealth tools is highly desirable.
• Knowledge of diverse specialties including psychiatry, endocrinology, cardiology, neurology, gastroenterology, and critical care practices.
• Demonstrated ability to perform comprehensive physical examinations and utilize diagnostic radiology tools such as CT scans when applicable.
• Strong understanding of clinical evaluation techniques like triage, wound care, suturing, and airway management.
• Excellent communication skills with a focus on patient service and health coaching; experience working in hospital or hospice environments is advantageous.
• Ability to manage complex cases involving acute pain management, post-operative recovery, or chronic disease conditions such as diabetes or rheumatology issues.
• Commitment to ongoing professional development through participation in clinical research or clinical trials; familiarity with CPT coding and ICD coding standards is preferred.
