Advanced Practice Provider (APP) – In-Home Chronic Care

About the Role:
We’re seeking Advanced Practice Providers to join a nationally recognized leader in in-home care for patients with chronic kidney disease and end-stage renal disease. This innovative model combines technology-enabled care management, medication review, and patient education to improve outcomes, reduce costs, and expand access to high-quality, evidence-based care.
A Day in the Life:
• Conduct comprehensive initial assessments, follow-ups, and hospital transitions
• Perform hands-on, head-to-toe assessments and obtain full vitals
• Manage medications and provide disease process education
• See approximately 4–5 patients per day
• Visit durations: 90 minutes for new patients, 30–60 minutes for follow-ups

Job Responsibilities: A Day in the Life:

Qualifications: Qualifications:
- Minimum 2 yearss' experience as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant)
- Experience in home health, primary care, internal medicine preferred
Working Hours: - Hybrid role: Full-time, Monday - Friday
- 3–4 days visiting patients in their homes, 1–2 days remote (for charting, telehealth, and administrative work)
- No weekends or on-call responsibilities
- Patients are typically scheduled within a clustered territory
- AI-driven scheduling to optimize daily routes and patient flow
