Job Description:
• Provides case management/therapy for patients and families to enhance their psychosocial functioning
• Performs duties and responsibilities in a manner consistent with mission, values and mercy service standards
• Join a virtual collaborative care team where learning, evidence-based practice and consistent communication drive better outcomes
• Ensures safety, confidentiality, regulatory compliance, and adherence to hospital and infection control standards
• Coordinates interdisciplinary care, advocates for patients and families, and oversees safe, comprehensive discharge planning
• Provides behavioral health case management, assessments, therapy, crisis intervention, and ongoing treatment planning with accurate documentation
• Communicates effectively with patients, families, and community partners while maintaining strong clinical, organizational, and technology skills
Requirements:
• Master's degree required in Social Work, Counseling or another human-service related field
• Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) in Missouri required
• Minimum 2-3 years Behavioral Health experience preferred
• Co-worker must be able to communicate orally, in writing and demonstrate proficient computer skills
• Co-worker must be able to listen and to understand and explain complex situations
