Scheduled: 10:00am - 6:00 pm - Remote
Salary Range: The salary range for this position is $75,000 – $90,000 yearly.

As an Integrated Care Lead in CBI Home Health’s specialized, integrated care programs, you will play a pivotal role in shaping seamless, patient-centered transitions from hospital to home, ensuring every individual receives the right care at the right time. Partnering closely with hospital teams, community partners, and families, you’ll create comprehensive care plans that not only support safe discharges but also empower patients to thrive sustainably at home.

What you will need to be successful:
Regulated Health Professional (Nursing, Occupational Therapist, Physiotherapist, etc.) in Ontario and possess a current registration with your regulatory therapy College
3 - 5 years of supervisory experience
Experience in home and community care or acute care (community experience is considered as an asset)
Excellent written and verbal communication skills
Ensure patient and family centric care
Strong organization skills and able to work in a fast-paced environment
Ensure frequent, effective, and timely communication between staff and external stakeholders (patients, families, referral sources, funders)
Be a great team player and work in highly collaborative and interdisciplinary team
Provide input into business development and/or service delivery model development opportunities
Possess a valid driver’s license with access to a reliable and insured vehicle
