- Directly supervise, manage, and oversee staff. Including hiring, firing and providing performance evaluations. Complete individual development goals and objectives.
- Provide services to consumers as necessary to assist the clinical team.
- Educate, counsel, evaluate and manage performance of the team.
- Maintain/update knowledge of Medicare, Medicaid, insurance, and program specific administered waivers.
- Coordinates assignment of the clinical team to consumers.
- Possesses an active license as a Licensed Social Worker, Licensed Independent Social Worker or Registered Nurse OR a bachelor’s degree in nursing, Social Services, or related field.
- Possesses 36 months experience in home and community-based service environment within the past 10 years. Direct service experience with the A&D/TBI Program, as well as assessment, care plan development and monitoring preferred.
- Adheres to the CareStar Rule in performance of job responsibilities.
- Understands and complies with CareStar Policies and Procedures.
- Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
