**Work Environment and Culture**
As a Remote Social Worker – Medical Case Management and Advocacy, you will work from the comfort of your own home, with the flexibility to work anywhere in the U.S. Our work hours are Monday-Friday, 8am-4:30pm Pacific Standard Time, with no weekends or holidays required. **Responsibilities**
As a Remote Social Worker – Medical Case Management and Advocacy, your responsibilities will include:
* Partnering with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
* Researching available options and resources for members with financial, transportation, safety, and housing needs
* Empowering members to make positive lifestyle and medical choices
* Working as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
* Conducting telephonic or electronic exchanges with members to assess their needs and develop plans to meet those needs
**Qualifications**
To be successful in this role, you will need:
* An active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
* 2+ years of experience in medical social work or case management
* A Master's Degree in Social Work
