**Job Summary**
We're seeking an experienced Licensed Social Worker to join our remote team as a medical social work and case management professional. In this role, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Responsibilities include researching available options and resources for members with financial, transportation, safety, and housing needs. This position works to empower members to make positive lifestyle and medical choices and works as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
**Responsibilities**
* Research available options and resources for members with financial, transportation, safety, and housing needs
* Collaborate with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
* Empower members to make positive lifestyle and medical choices
* Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
* Conduct telephonic or electronic exchanges with members to assess their needs and develop plans to address them
* Develop and implement plans to address members' needs, including referrals to community resources and services
* Maintain accurate and up-to-date records of member interactions and progress
* Participate in team meetings and training sessions to stay current on best practices and industry developments
**Requirements**
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence
* 2+ years of experience in medical social work or case management
* Masters degree in social work or a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Strong analytical and problem-solving skills
* Ability to maintain confidentiality and handle sensitive information
* Familiarity with electronic medical records and other health care software systems
**Preferred Qualifications**
* Experience working with diverse populations, including children, adults, and older adults
* Experience working with members who have complex medical and social needs
* Familiarity with community resources and services, including housing, transportation, and financial assistance programs
* Experience working in a health care setting, including hospitals, clinics, and home health care agencies
* Certification in medical social work or case management
