The Clinical Consultant Pharmacist (CCP) is responsible to draw from
geriatric-focused clinical practice guidelines and collaborate inside
multidisciplinary teams to deliver clinical recommendations and process-guidance
for a portfolio of residential care, assisted living, and independent living
homes to optimize for resident outcomes, safety, and regulatory compliance. The
compensation is salary. The environment is hybrid work-from-home and on-site at
the care homes.
The Clinical Consultant Pharmacist works in partnership with the clinical team,
operations, home partners, and other departments in the CareRx organization.
This position is 4 days per week maternity leave (18 months) and is located in
Regina and Moose Jaw. The Clinical Consultant Pharmacist is responsible for
completing clinical pharmacy activities in alignment with the Saskatchewan
College of Pharmacy Professionals regulations, home service agreements, and
internal CareRx policies.

Role Accountabilities:
• Act as a liaison between long term care homes and retirement communities and
CareRx Pharmacy to ensure optimal Resident care, Pharmacy Services, and
Clinical support
• Travel to Special Care Homes and Personal Care Homes
• Build and maintain relationships within the community to ensure ongoing
collaboration and communication between home/community leadership teams and
pharmacy teams to develop/maintain strong partnerships
• Complete medication reviews in currently serviced retirement communities and
long-term care homes serviced by CareRx Pharmacy
• Complete all required documentation and billing processes as required through
SPDP, the Saskatchewan College of Pharmacy Professionals regulations, and
other standards and regulations
• Provide education to Residents and staff on various health-related topics
• Support CareRx Pharmacy policies and procedures within retirement communities
and long-term care homes
• Ensure regular on-site visits are made to all assigned clients
• Participate in the development and implementation of industry-leading
clinical pharmacy service, when applicable
• Be available for special clinical team projects as required
• Works collaboratively with other team members to support applicable homes and
communities
• Help with pharmacy operations/drug distribution when required
• Attend and actively participate in clinical team meetings
• Coordinate and actively participate in seasonal vaccination activities (fall
and spring)
What You Will Bring to The Team:
• Insured and currently licensed in good standing with the Saskatchewan College
of Pharmacy Professionals
• Proven ability to foster strong partnerships with other members of the
healthcare team
• Effective organization, planning, and time management skills
• Knowledge of Kroll Windows application and Microsoft Office programs
• Must be familiar with the provincial standards of practice established for
SCH and PCH
• Excellent customer service and interpersonal skills, oral and written
communication skills and be able to interact with persons of varying levels
of professional expertise
• Excellent time management skills
• Skilled in critical thought processes and be able to prioritize appropriately
• Must demonstrate initiative and creativity
• Self-directed requiring minimal supervision and able to work independently
• Motivated to achieve results that reflect a high standard of quality

Location: Regina and Regina-Area (Moose Jaw)
Work Shifts: This is a full-time position working 4 days a week, with
flexibility and opportunity to work-from-home and on-site from the care homes.
The successful candidate will be required to work 1 day per week in CareRx
Regina (operations) and the remaining 3 days per week in the clinical
department, with an emphasis on medication reviews (SMAPs and non-SMAPs).
Opportunity: This is an 18 month maternity leave position, Sign On Bonus subject
to terms and conditions discussed with the successful external applicant.
