Duration: 9 month contract with possible extension
Location: 100% Remote
Responsibilities:
Member Outreach & Engagement
• Run outreach campaigns to members with open care gaps (e.g., colorectal cancer screening, breast cancer screening, A1c control, blood pressure management, vaccination)
• Conduct multi-channel outreach (phone, SMS, email, mail, and/or portal) to education members, address barriers, and support completion of recommended care.
• Schedule appointments with PCPs, NPs/PAs, or specialists to facilitate gap closure
• Address common barriers to care (transportation, language, health literacy) and connect members to appropriate resources
• Send follow-up communications (letters, portal messages) to unreachable members per standard workflow
• Document all member interactions accurately and in a timely manner per HIPAA standards
• Use approved scripts and job aids while applying clinical awareness within the Medical Assistant scope.
Quality Measure & Data Support
• Enter diagnostic results, lab values, and screening documentation into EMR to ensure compliance with quality measures
• Track open and closed care gaps in EMR and outreach tracking tools; update member statuses after each encounter
• Pull and reconcile records (e.g., immunizations, external labs) from external systems to support gap closure
Care Coordination & Team Collaboration
• Collaborate with clinical leadership, care managers, and providers to coordinate timely completion of needed services
• Escalate complex member needs to clinical staff as appropriate
• Support other population health and operational initiatives as assigned
Required Qualifications:
• 1+ year of HEDIS experience required
• Active Certification as a Certified Medical Assistant required
• 1+ year of experience in population health, care management, or a related setting
• Excellent member-facing communication skills, with the ability to explain clinical concepts in clear, accessible language.
