Our client is seeking a compassionate and dedicated Case Manager to join their fully remote Community & Social Care team. This vital role involves providing comprehensive support and advocacy for individuals and families navigating complex social service systems. You will be responsible for assessing client needs, developing personalized service plans, coordinating access to resources, and monitoring progress towards goals. The ideal candidate possesses strong empathy, excellent communication and organizational skills, and a thorough understanding of social welfare programs and community resources. This is a remote-first position, allowing you to make a significant difference in the lives of vulnerable populations, primarily serving those within or connected to the **Colorado Springs, Colorado, US** community, from the comfort of your own home. Key Responsibilities: Conduct comprehensive assessments of client needs, strengths, and challenges. Develop individualized care plans in collaboration with clients and their families. Coordinate and facilitate access to essential services, including housing, healthcare, employment assistance, mental health support, and legal aid. Provide ongoing support, counseling, and advocacy to clients. Monitor client progress and regularly review/update care plans as needed. Maintain accurate and confidential client records in compliance with agency policies and legal requirements. Liaise with community agencies, service providers, and government entities to ensure seamless service delivery. Educate clients about available resources and assist them in navigating bureaucratic processes. Respond to crisis situations and provide immediate support and intervention. Participate in team meetings, case conferences, and ongoing professional development. Qualifications: Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field. Master's degree preferred. Minimum of 3 years of experience in case management, social work, or a related client-facing role. Thorough knowledge of social service programs, community resources, and relevant legislation. Excellent assessment, interviewing, and active listening skills. Strong empathy, patience, and interpersonal skills. Exceptional written and verbal communication skills. Proficiency in case management software and standard office applications. Ability to work independently and manage a caseload effectively in a remote setting. Strong organizational and time management skills. Commitment to ethical practice and client confidentiality. Experience working with specific populations (e.g., families, elderly, individuals with disabilities) is a plus.
